During the rescue, seven people were poisoned and suffocated, causing an accident. Four people including the manager died. According to preliminary analysis, the cause of the accident was the poisoning of personnel caused by the release of adsorbed toxic and harmful gases from the hydrolysis protective agent, insufficient risk identification, incomplete preventive measures and improper rescue measures during the operation.
At 0:40 on January 15, a worker at the silo of the phosphoric acid workshop of Jiyuan Fengtian Fertilizer Industry Co., Ltd. accidentally fell into the silo during the operation and died after being sent to the doctor to fail.
The two accidents exposed problems such as inadequate management of special operations by the accident company, insufficient professional skills of employees, irregular use of protective equipment, and a serious lack of self-protection awareness.
